Veterans, their spouses, or their dependents who wish to enroll but have never filed for education benefits must complete an application for VA Education Benefits. 

Veteran (Chapters 30, 31, and 1606) VA Form 1990
Dependent or Spouse (Chapter 35) VA Form 5490

Veterans, their spouses, or their dependents who are currently receiving benefits and wish to change colleges or academic major should complete one of the following:

Veteran VA Form 1995
Dependent or Spouse VA Form 5495
