I understand that by submitting this form I am certifying that I have applied for Financial  Aid for this semester.  The amount of these charges will be withheld from any Financial Aid Award or Scholarships applied to my account. I also understand that if my these awards do not cover the complete cost of these charges I will be responsible for payment. 

Refunds are given only if you drop a class or the class has been canceled.  Proof of purchase in the form of  a cash register receipt or a copy of the Financial Aid Charge, and a completed drop form is required to receive a refund.  Refunds will not be given if you find a cheaper book, or decide you will not be needing the book.  On-line shipping costs are not refundable.
