Check the Transfer Advisement calendar for an available appointment date.

Fill out the appointment request form below. Be sure to include all information required. Once the form is completed submit it back to us.
Step #3

Once we receive your form one of our advisement specialists will be in contact with you regarding your appointment.
Step #4

Attend your appointment. If you cannot make your scheduled appointment be sure to contact your advisor.
